About the role
- This role is full-time and fully benefitted at 32 hours/week with a schedule of 4 days with 8-hour shifts.
- This has the following schedule, based on a 2 week rotation: Week 1: Monday, Thursday, Friday, Saturday; Week 2 is: Monday, Tuesday, Wednesday, Thursday.
- Please note: there is one shift every other week that will be scheduled at our Chula Vista Health Center.
Responsibilities
- Partner with a Case Management team, which will communicate with patients on lab results, referrals, and other aspects of patient care.
- This role is designed for licensed Nurse Practitioners, Physician Assistants, and Certified Nurse Midwives licensed in the state of California.
