Responsibilities

  • Lead High-Performing Teams
  • Recruit, onboard, coach, mentor, and develop team members.
  • Drive Operational Excellence
  • Build and maintain dashboards, scorecards, and reporting tools that monitor:
  • Lead daily operational huddles focused on productivity, quality, and workload management.
  • Develop scalable workflows and operational processes that improve consistency and outcomes.
  • Recognize high performers while coaching employees needing additional support.
  • Implement performance improvement plans and corrective action when appropriate.
  • Maintain compliance with CCM documentation requirements and payer guidelines.
  • Partner Across the Organization

Requirements

  • 7+ years of progressive leadership experience managing high-volume, metrics-driven teams.
  • At least 3 years leading people managers or large operational teams.
  • Strong analytical skills with the ability to translate data into operational improvements.
  • Experience using EHR systems, care management platforms, and reporting tools.

Nice to have

  • Master's degree (MBA, MHA, MSN, or related field).
  • Clinical licensure (RN, LCSW, or similar) is a plus.
