Responsibilities
- Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
- Coordinates with the broader care team to ensure the patient has the support they need.
- Our care model combines in-clinic and virtual care to bring support to patients where and when they need it.
Requirements
- You have an active, unrestricted license to practice in California, or willingness to obtain.
- You have excellent communication and interpersonal skills.
- You have high emotional intelligence and know how to make a patient feel at ease - without judgment and without shame.
Nice to have
- experience in chronic condition management and weight management preferred.
- At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine

- Provide clinical guidance to medical assistants and nursing teams as assigned.
