About the role

  • Nursing Full-time position available performing direct patient care and patient education in Senior Care Options Department (SCO).
  • Will facilitate initial enrollments into the SCO Program as well as coordinate care with nursing, case management, social workers, and rehabilitation department to coordinate care for Skilled Nursing Facilities.
  • MA RN license and minimum one year working with geriatric population.
