Responsibilities
- Serve Together, Provide Value, and Deliver Exceptional Quality Care.
- You'll complete comprehensive psychosocial assessments, provide crisis intervention and counseling, and support advance care planning conversations that help families make informed, values-based decisions.
- Your work will directly support both patients and families while also strengthening the care experience across the entire hospice journey.

Requirements
- Master's or doctoral degree from a CSWE-accredited school of social work
- Minimum 1 year of experience as a licensed Master's Social Worker
Nice to have
- Hospice or palliative care experience
- Clinical experience with grief counseling and end-of-life issues
- Expertise in psychosocial assessment and crisis intervention
- Ability to guide patients and families through end-of-life challenges
- Excellent communication skills with emotional intelligence
- Cultural sensitivity and competence working with diverse populations
- Knowledge of community resources and social service systems
- Strong interdisciplinary collaboration abilities
Compensation
- $70,000 - $75,000 USD
