Responsibilities
- Performs and documents patient's screenings, psychological assessments, progress and referrals for service members, veterans and military family members.
- Collaborates with clinical team to perform risk management and crisis intervention services as necessary.
- Develops network of trusted contacts within organizations in which to refer and coordinate care.
- Collaborates and coordinates with the caregiver, physician, clinic director, case management and all other disciplines to provide the best care to patients as needed.
- Consults with service providers and community partners on resource-related issues.

Requirements
- Master's Degree in Social Work, Sociology, Psychology or related field required.
- Years of Experience
- Two or more years in a customer service focused environment required.
- 1+ year's behavioral health experience required; 3+ years preferred. 2+ years in a customer service focused environment.
- One or more year's behavioral health experience required; three years preferred.
Nice to have
- 3+ years preferred.
- 2+ years in a customer service focused environment.

- Military background or prior experience in serving the military community and/or veteran administrative organizations preferred.
Skills
- Effectively communicate via written, verbal, in person and virtual methods.
- Knowledge of and experience working with local social service, public health, and VSO organizations and resources.
- Understanding of a range of treatment modalities to include child, couples, and family treatment is preferred.
- Ability to establish and maintain effective working relationships.
- Ability to maintain confidentiality.
- Excellent written and oral communication skills.
- Must be highly organized, detail oriented, and reliable.
- Capacity for flexibility and adaptability in different work situations.
- Ability to work well with diverse populations.
- Strong consultative abilities and conflict-resolution skills.
- Compliance requirement for annual completion of KPI forms
- Outstanding organizational, interpersonal and communication skills
Compensation
- $50,700.00--$71,100.00
