Responsibilities
- Collaboration- With so many opportunities to build processes and improve how we deliver care to our patients, we rely heavily on one-another to communicate changes effectively to ensure seamless support across functions.
- Provide coaching and development support for APPs, fostering clinical excellence and a strong team culture.
- Support quality improvement initiatives and ensure adherence to clinical guidelines and value-based care metrics.
- Build and maintain collaborative relationships with community partners, payers, and referring providers.

- Work with the Medical Director to ensure clinical accuracy, identify and support clinical training.
- Collaborate with the market VP/General Manager and Health Services Director as part of the leadership triad and other Pod Leaders on market workflows, quality improvement initiatives, and to drive outcomes
- Use Nest Health's electronic health record and care coordination tools for documentation, tracking, and communication.
- Active and unrestricted Medical Director, Nurse Practitioner or Physician Assistant License in Louisiana with national board certification (FNP, PNP, or similar)
