- Champion engagement by raising awareness of mental health services through client-driven wellness initiatives, proactive check-ins, workshops, and educational programs.
- Identify and address social determinants of health (like access to food or transportation), bridging the gap to necessary community resources to enhance overall well-being.
- Cultivate strong, collaborative relationships with health center staff and leadership, adapting to evolving center and patient needs.
- A Master's in Social Work (M.S.W.) from a CSWE-accredited program-or a related behavioral health field.
- At least 12 months of recent (within the last 2 years) experience providing evidence-based individual counseling in a mental health or healthcare setting.
