About the role
- This role may be full-time or part-time depending on market needs.
- At Matter Health, our APPs are more than providers - they are Panel Owners.
- This is a fully community-based role with no traditional clinic home base.
Responsibilities
- You will manage a dedicated panel of older adults, serving as their longitudinal primary care provider while building trusted relationships directly within the communities you serve.
- Basic Life Support (BLS) certification
Requirements
- Experience with EHR documentation (athenahealth experience preferred)

- Maintain accurate and timely EHR documentation and coding specificity Qualifications
- Active, unrestricted Nurse Practitioner or Physician Associate license
