About the role

Primary Care Physicians provide daily care to patient panels capped at approximately 500 patients.
This model emphasizes work-life balance while delivering a concierge-like experience for patients.

Responsibilities

Deliver Patient-Centered Care
Perform thorough assessments and develop personalized care plans for geriatric patients.
Manage chronic conditions, preventive care, and acute issues within the scope of value-based care principles.
Utilize evidence-based guidelines to optimize patient outcomes and quality metrics.
Collaborate with your team and specialists to ensure seamless care delivery.
Lead care plan discussions and participate in team meetings to review patient progress.

Requirements

2-6 years of experience in a primary care setting, preferably managed care, or recent residency graduate.

Nice to have

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
Board certification or board eligible in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.

Our contracts involve three components: a base salary, a qualitative performance bonus, and beginning in your third year of employment, a clinic performance bonus.
Your compensation in the first two years should approach $300k, with the opportunity to exceed $400k including your clinic performance bonus.
