About the role
- Integrate strategies into program plans. - Ensure requests for services are made considering eligibility, continued stay and discharge criteria for the service definition.
- Schedule and participate in on-call coverage.
- Specific offerings are tailored to the role, employment type, and location.
Responsibilities
- Provide initial and ongoing assessment, evaluate effectiveness of person-centered plan and progress made by person receiving services, participate in team meetings with other providers and natural supports as needed and facilitate development of person-centered plan.
- Continuously use person-centered approaches and positive approach strategies when interacting with people we support. Integrate strategies into program plans.
- Ensure requests for services are made considering eligibility, continued stay and discharge criteria for the service definition. Services and support that match the person-centered plan and the individual's needs shall be in place and be fiscally viable.
- Maintain a safe working environment for employees and people receiving services.
- Additionally, the role supports a safe and therapeutic environment through crisis response, incident management, case management, care coordination and collaboration with individuals served, families, and interdisciplinary teams.
Requirements
- Associates (Required), Bachelors
- You will be required to follow any other instructions and to perform any other duties and responsibilities upon the request of a supervisor.
Compensation
- Ensure that all required and/or requested reports/documentation are completed. Maintain the records of the individuals supported. Coordinate the completion of daily, monthly, quarterly, and annual documentation in the EHR and/or medical record.
