About the role

Sabathani Community Center's Health Equity Department works to reduce health disparities and improve access to care for community members through direct service, care coordination, and community education.
Sabathani Community Center is seeking a contracted Public Health Registered Nurse to conduct comprehensive initial assessments for families newly enrolled in Sabathani's Non-Medical Family Home Visiting program.
The contracted PHN plays a critical clinical role in the program, administering validated screening tools, identifying family strengths and needs, and ensuring families with urgent safety or mental health concerns receive an immediate, appropriate response.

Knowledge, Skills and Abilities: ● Ability to build rapport quickly and deliver assessments in a culturally responsive, trauma-informed manner. ● Sound clinical judgment, including the ability to recognize and respond appropriately to safety concerns. ● Strong documentation skills and ability to meet agreed-upon reporting timelines. ● Ability to work independently while maintaining consistent communication with program staff.
The department's programming is designed to meet residents where they are, with a focus on culturally responsive, trauma-informed care.

Qualifications Education & Licensure: ● Active, unencumbered Registered Nurse (RN) license in the State of Minnesota required. ● Public health nursing experience or certification preferred.
