- PRIMARY RESPONSIBILITIES Assist the Delegating Nurse in prioritizing and responding to client needs according to the Plan of Care.
- Follow the individualized Plan of Care as written by the Delegating Nurse.
- Provide mobility and transfer assistance as needed.
- Assist with daily routines, light housekeeping, and maintaining a safe home environment.
- Communicate and actively listen to client needs and concerns while promoting dignity and independence.
