About the role

The Supportive Housing Case Manager will work with a caseload of 10-20 single and family households in supportive housing.
The clients in these programs experience a number of concerns from mental health diagnoses, substance abuse, physical/developmental disabilities, child protection involvement, limited income, domestic violence, and chronic or long-term homelessness.
The Case Manager will guide both single and family households to services and resources to help them stabilize, rebuild their lives, and graduate out of the Supportive Housing program.

He/she will support the housing first model with a trauma informed care approach that will maximize the likelihood of client success and long-term self-sufficiency.
