About the role

- Note: The Centers for Medicare & Medicaid Services (CMS), in collaboration with the Centers for Disease Control and Prevention (CDC), require COVID-19 vaccinations for all Medicare and Medicaid certified providers.
- You'll provide home-based nursing care for patients as directed by the attending physician and by following the order sets and goals created by the Case Manager.

Nice to have
- Minimum one-year experience as a LPN in an acute care setting.
- Home care experience preferred.
