About the role

As a Referral Coordinator, you keep patients moving through their care journey.
You're the connector between patients, providers, and specialists, ensuring seniors get the care they need without delays or confusion.
This is detail‑driven, people‑first work where your organization, communication, and follow‑through directly shape patient outcomes.

Responsibilities

Document thoroughly in the EMR and maintain clean, accurate patient files
Respond to patient and provider inquiries with concierge‑level support
Partner with providers to create seamless patient experiences
Collaborate with clinical staff to keep care coordinated
Support front office and MA workflows when the clinic needs an extra hand
Maintain a patient‑first mindset while upholding clinic standards
Bring warmth, clarity, and concierge‑level support to every patient interaction

Requirements

Brooksville clinic (some local travel may be required to help cover other centers).

Nice to have

2+ years managing referrals from primary care to specialists
The ability to multitask, prioritize, and stay organized in a fast‑paced clinic
Compassion, professionalism, and a patient‑first mindset
High attention to detail and commitment to excellence
