About the role
- Demonstrate an excellent understanding of oppression through an intersectional lens, including but not limited to discrimination, racism, Anti-Black racism, ableism, disablement, Islamophobia, xenophobia, and the impacts of poverty and social exclusion.
- Strong communication skills and experience applying social justice and health equity frameworks are essential.
- Assess patients to determine the need for service, identify psychosocial concerns, and formulate recommendations, treatment plans, or intervention strategies.
Responsibilities
- The Social Worker for UHN's Blood Disorders Program will primarily support adult outpatients in the Red Blood Cell Disorders Clinic, including patients living with hemoglobinopathies such as Sickle Cell Disease and Thalassemia.
- The successful candidate will work collaboratively with an interdisciplinary team to provide psychosocial assessment, counselling, advocacy, system navigation, resource coordination, and support for patients and families.
- Collaborate with a core interdisciplinary team to support patients living with hemoglobinopathies, including Sickle Cell Disease and Thalassemia.
- Develop, implement, reassess, and modify intervention plans in collaboration with patients, family members, significant others, and the interprofessional team.
- Provide counselling, advocacy, crisis support, care coordination, and system navigation to support patients' psychosocial functioning and overall care.
- Establish and maintain communication with members of the interprofessional team and community agencies to obtain, provide, or exchange information related to patients' psychosocial functioning and care needs.
Requirements
- Completion of a Master's Degree in Social Work required
- Knowledge of policies, legislation, program's and issues related to social welfare institutions and services
- Knowledge of a second language an asset
Nice to have
- Demonstrated experience in working within strengths-based, empowerment, and trauma-informed approaches preferred.
Skills
- Excellent advocacy skills in a social justice framework, with demonstrated experience supporting clients facing marginalization
- Excellent interpersonal, team work, time management, organizational, and problem solving skills
- Excellent writing skills
Compensation
- $43.47 - $54.34 per hour
