About the role

  • Reports out monthly on the impact of outreach services utilizing tools created by the PFK data team, claims data, Epic, and data pulled from the community provider EMR.

Responsibilities

  • Identifies innovative outreach opportunities; collaborate with PFK team to design and pilot new outreach methods.
  • Collaborates with PFK Quality Improvement, Pharmacy Team, and Provider Relations on outreach efforts and other support opportunities associated with their project work.
  • Serves as a liaison with NCH marketing to design and order outreach materials. Develops and maintains a standard collection of outreach materials.
  • Develops tracking resources to support member identification and outreach impact.
  • Coordinates, tracks, and implements outreach to patients and families on behalf of community providers that serve PFK members.

Requirements

  • Computer skills, Decision Making, Driving motor vehicles (work required) *additional testing may be required,

Skills

  • Working knowledge of Medicaid and other regulatory agency standards.

Compensation

  • $11k-$20k
