Responsibilities

  • Chronic Disease Management: Proactively manages conditions such as diabetes, hypertension, anticoagulation, and asthma to improve control and prevent complications.
  • Collaborative Practice & Protocols: Works directly with physicians and providers to implement and follow care protocols, including independent medication adjustments when appropriate.
  • Administrative & Support Activities: Assists with physician consultations, provides accurate drug information, and supports the department's quality improvement initiatives.
  • Performs other duties as assigned and modified at manager's discretion.

Requirements

  • Doctorate of Pharmacy (PharmD) required
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employment

Nice to have

  • Valid pharmacy license in good standing Registered Pharmacist (RPh) required, Doctor of Pharmacy (PharmD) preferred
  • Pharmacy residency or ambulatory care certification (e.g. BCACP) preferred
  • Certification in immunization and medication therapy management is a plus
  • Residency training or ambulatory care certification is highly preferred
  • Experience in managed care, HMO, PBM, or MSO settings is a plus

Skills

  • Knowledge of and experience with state and federal insurance programs
  • Familiarity with appropriate local, state, and federal pharmacy-related regulations
  • Excellent organizational, communication, and clinical decision-making skills
  • Willingness to travel locally up to 50% of the time; flexible to work evening, weekends and/or holidays as needed
  • Fluency in spoken and written English
  • This job requires use and exercise of independent judgment

Compensation

  • $118,062 - $168,658 Salary The posted pay range represents the base hourly rate or base annual full-time salary for this position.
  • Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors.
