About the role
- Concord Hospital Medical Group is seeking an experienced APRN or PA-C to join an established outpatient family medicine practice at Pembroke Family Physicians.

Responsibilities
- Diagnose and manage acute and chronic conditions
- Document care accurately to support continuity and quality
- Collaborate closely with physicians, APPs, and clinical team members
- Material Handling: Light work - regularly lifts/pushes/pulls Physical Activities: Frequently bends, uses fine motor skills, hears, reaches, sits, speaks, and walks; occasionally performs repetitive motions, squats, or stands.
- Established practice with strong interdisciplinary support
Requirements
- Employees must be able to perform the essential functions of this role.
- APRN: Master's or Doctoral degree from an accredited Nurse Practitioner program
- PA-C: Master's degree from an ARC-PA-accredited Physician Assistant program
Nice to have
- Minimum of 3 years of outpatient primary care experience preferred
