About the role

  • The Specialty care coordinator serves as a subject matter expert for resource navigation, and onboarding/training needs.
  • This role reports to the Manager/Supervisor.
  • Essential Duties and Responsibilities Schedules & facilitates patient case reviews; Distributes patient list for review and recommendations to the care team after reviews.

Responsibilities

  • We maintain a drug-free workplace and perform pre-employment substance abuse testing and background verification checks.
  • Supports quality improvement activities through gathering, tracking, and reporting data as directed by the manager/supervisor and ROM lead.

Compensation

  • New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year New York Non-exempt: New York City and Long Island: $17.00/hour, Nassa
