Responsibilities

- This position assesses Oregonians applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need.
- Case Managers provide choice counseling and help establish, develop, and implement case management plans, coordinate the provision of services for in-home, community-based care and nursing home care, regularly review and update long-term care service eligibility, monitor social service plans, and identify and mitigate safety risks.
- Develop detailed, individualized, and person-centered case planning for Oregonians.
Requirements
- A bachelor's degree in a Behavioral Science, Social Science, or a closely related field
- Experience interpreting and applying laws, regulations, policies, or procedures to make consistent, accurate, and well-supported decisions.
- Experience conducting interviews or information-gathering conversations to assess needs, evaluate available information, and determine appropriate next steps using sound judgment as a case manager.
