About the role
- This role assists in connecting neighbors to services by coordinating referrals, scheduling appointments, suppo rting documentation, arranging transportation, and helping ensure timely follow-up with service providers.
- Work ing collaboratively with care team members, the Referral Coordinator helps remove barriers to care and promotes efficient service delivery, so Care Coordinators can focus on direct engagement and case management.
- Provide reminder calls, messages, and follow-ups to ensure neighbors attend scheduled appointments and services.
Responsibilities
- The Referral Coordinator provides administrative and service coordination support to Care Coordinators.
- Communicate regularly with Care Coordinators, and Community Supports staff to ensure continuity of care and timely ser vice c oordination.
- Maintain organized records and referral tracking systems to ensure compliance with program and documentation standards.
- Provide administrative and logistical support for care coordination activities, including preparing materials and ass isting wi th follow-up communication.
- Support outreach and engagement efforts by helping coordinate appointments, initial contacts, and referral processes.
Requirements
- High school diploma or equivalent required.
- Experience scheduling appointments, coordinating referrals, or supporting care teams.
