- As the Cardiology Patient Transition Coordinator, you will play a key role in supporting patients during the transition of our cardiology clinic.
- This position focuses on providing compassionate, knowledgeable, and efficient assistance to help patients navigate their next steps in care.
- You will guide patients in identifying appropriate providers within our organization or in the broader community, ensuring continuity of care and a positive patient experience.
Responsibilities
- Maintain confidentiality and comply with HIPAA and other relevant regulations.
- Maintain up-to-date knowledge of available providers and services in the region.
Requirements
- Ability to work independently and collaboratively in a fast-paced, transitional environment.
- Experience in working with insurance plans and understanding in-network vs. out-of-network referrals.
