About the role
- Our Devoted Medical Specialty Care Advanced Practice Provider (APP) roles include both Nurse Practitioners (APRNs) and Physician Assistants - candidates for both will be considered.
- In this role you will be working in a next generation virtual specialty clinic that dramatically expands access to care for America's most vulnerable seniors.
- The clinic focuses on optimizing hypertension management and reducing cardiovascular risk for Devoted Health members.
Responsibilities
- Conduct focused and thorough assessments of patients with hypertension through virtual consultations including ordering diagnostics as needed, interpreting labs and imaging data, and developing a treatment plan in collaboration with the specialty care clinic team.
- Formulate accurate diagnoses and develop individualized treatment plans for patients, including medication management, lifestyle modifications, and monitoring recommendations.
- Serve as the clinical advisor and provide clinical escalation support for the specialty clinic staff and other teams during business hours.
- The Cardiometabolic clinic is one of several of Devoted's virtual specialty care programs that are designed as "micro centers of excellence" that deliver highly tailored, specialized care to patients with specific chronic conditions.
- You're comfortable operating in a build environment where not everything has a playbook yet - and that energizes rather than frustrates you.
Requirements
- Multi-state licensure is required, along with the ability and willingness to obtain and maintain additional licenses as needed.

- Ability to work Monday-Friday 8:30am-5:00pm MST or PST preferred

Skills

- Familiarity with AI-assisted clinical workflows
- Proficiency in using telehealth technology and electronic health records (EHR).
- Proficiency in English and Spanish preferred for this position.

- The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting.

- You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits.
- Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface.
- Participate in regular panel review discussions to offer advice and provide guidance around medical management.
- Maintain accurate and up-to-date patient medical records, ensuring compliance with relevant legal and ethical guidelines.
