Develop and revise individualized plans of care based on patient status and evolving needs.
Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
Provide counseling and support to patients and caregivers before, during, and after treatment.

Preferably 2+ years of nursing experience, with a focus on geriatrics.
Nursing degree from an accredited institution.

Full Time Schedule: Monday - Friday Weekend and Holiday Rotation - No traditional on-Call Salary Range: $74,000.00 - $90,000.00 Responsibilities: Initiate appropriate preventive and rehabilitative nursing procedures.
Perform thorough physical assessments and gather comprehensive medical histories.
