About the role
- This role combines clinical assessment, care coordination, patient and Personal Care Attendant (PCA) education, and quality assurance to ensure safe, effective, and person-centered care.
- Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance.
- The Case Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes.
Responsibilities
- Develops, implements, manages patient's activity plan(s).
Requirements
- Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed.
- Performs other job-related duties as may be required or directed by the supervisor.
Nice to have
- Home care case management
- Care coordination experience
- Valid and Active Registered Nursing License issued by the Oregon Board of Nursing
- Ability to articulate cases through effective summaries.
- Ability and flexibility to travel (usually by car) within assigned geographic area
- valid driver's license and car insurance required.
Compensation
- $45k-$50k
