About the role

The Case Manager provides direct physical and supportive services to the young people we serve in Covenant House New York's short-term emergency Youth Shelter, in a positive and nurturing manner, using an integrated, multidisciplinary team approach.

Collaborates with youth to assess strengths and challenges and develop an individual service plan that reflects the youth's progress and stated goals.
Creates and maintains confidential records, charts, and logs for youth, documenting all relevant information from individual and group interventions and with collateral services.

Reporting to CHNY's Program Coordinator, the Case Manager will support a culture that successfully achieves meaningful results for the young people we serve.

Strong multi-tasking skills with demonstrated ability to manage multiple projects at once required.
Excellent verbal, written, and problem-solving abilities and an upbeat demeanor are required.
A strong focus on child protection and ensuring the safety of residents is required.
Experience working with diverse groups is required.
Overall flexibility in work schedule required, including weekends, evenings, and holidays if needed.

Bachelor's degree preferred.
If no Bachelor's degree is required, High School or GED is required, with one (1) year of experience in a residential setting working with adolescents preferred.
