  • Collaborates with advanced practice clinicians and/or social worker to develop an action plan

Requirements

  • Required: High School Diploma or GED.
  • Ability to communicate positively, professionally and effectively with others
  • Problem-Solving Skills: Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.
  • Required: Current and valid California Driver's License and reliable transportation.
  • While performing the duties of this job, the employee is regularly required to talk or hear. The employee regularly is required to stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls; and reach with hands and arms.

Nice to have

  • Senior care experience.
  • High School Diploma or GED.
  • Bachelor's Degree

Skills

  • Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise
  • Knowledge of and experience with applying the stages of change or a similar behavior change model
  • Able to work independently and remotely.
  • Proficient computer skills including Microsoft Office Excel, Word and PowerPoint
  • Ability to work a flexible schedule
  • Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.

Compensation

  • $48,373.00 - $72,559.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.

  • Contacts patients that are eligible for Alignment's Care at Home program and provides necessary wellness coaching to reduce or eliminate high-risk behaviors
  • Assists patients with accessing community resources and social services to address social determinants impacting health and wellbeing
  • Develops and distributes health education materials to identified patients, caregivers, and/or families
  • Coordinates Care with other Alignment team members, including health-related vendors for optimal patient care
  • Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations and AHP policies
