About the role
- The AHT Case Manager will be responsible for providing psychosocial services including but not limited to: screening, outreach, risk/psychosocial assessment, crisis intervention, supportive counseling and psychoeducation.
- The AHT Case Manager will participate in the implementation of evidence-based practices including Motivational Interviewing and Safe Crisis Management.
- They will be responsible for co-facilitating psychoeducational groups with survivors, as well as preventive groups to youth in CHNY's general programming.
Responsibilities
- Engage at least weekly with clients residing in CHNY Bronx programs to provide support in working towards their ISP goals developed with their residential program assigned Case Manager.

Requirements
- Participate in all required training, supervision and staff meetings.
- Bachelor's degree required.
- Ability to monitor and de-escalate during crisis situations.
- Ability to follow instructions, prioritize assignments, and work in a fast-paced environment.
Nice to have
- One (1) year experience in a residential setting and working with adolescents preferred.
- Experience working with survivors of human trafficking and/or at-risk youth strongly preferred.
Skills
- The Bronx/Community AHT Case Manager will be responsible for inputting data into AHT internal and external online databases.
- Knowledge/Skills Strong interest/passion for working with homeless and runaway youth.
Compensation
- $16k-$24k
