About the role

Meet with assigned families regularly and provide appropriate services and referrals geared towards housing stability.
Ensure that clients achieve the objectives of maintaining long-term housing, by complying with the terms of their lease agreement and have an Individualized Housing Sustainability Plan that helps them achieve their desired level of self-sufficiency.
Mediate household, roommate, and/or landlord-tenant disputes that threaten housing stability.

The Case Manager will support the housing first model with a trauma-informed care approach that will maximize the likelihood of client success and long-term self-sufficiency.
The Case Manager will serve as the primary point of contact for all assigned client services and provide guidance and support to clients working toward self-sufficiency.
Support clients in the creation and maintenance of financial budgets and support clients in implementing money management techniques.
Participate in meetings with representatives from other agencies including Faith-Based and Community Based Organizations to deliver wrap-around services to clients.
Serve as point-of-contact for landlords and maintain positive landlord relationship.
Develop and maintain a Resource Directory that includes state and local service providers.
Maintain thorough case documentation in HMIS and client files and comply with agency and standard best practices.

Bachelor's Degree in Social Work, Psychology, Human Development, or related degree or commensurate experience.
Three years of experience in service delivery with low and moderate-income families.
Ability to prioritize competing priorities and make sound judgment.
Ability to advocate, assist in problem-solving, and achieve results for families Excellent time management and organizational skills Valid Driver's License Willingness to work evenings and weekends as needed.
Required: Bachelor's Degree in Social Work, Psychology, Human Development, or related degree or commensurate experience.
