About the role
- Practice at the highest level of your clinical expertise.
Responsibilities
- Develop, implement, and evaluate individualized resident care plans.
- Support smooth transitions for residents returning from hospital.
- Partner with nursing leadership to support staff performance and probationary reviews.
- Lead quality improvement initiatives and clinical best practice implementation.
Requirements
- Master's Degree in Nursing (MScN).
- Demonstrated experience practicing as a Nurse Practitioner or extensive experience in geriatric or long-term care settings.
- Knowledge of current legislation, best practices, and standards related to Long-Term Care.
- Working Conditions Travel is required between multiple Long-Term Care Homes within the Greater Toronto Area.

Nice to have
- Experience in Long-Term Care or geriatrics.
- Gerontological Nursing certification.
- Fluency in Chinese is considered a strong asset.
