About the role
- Conveniently located in the North Bay, this hospital consists of 144 acute psychiatric inpatient beds.

Responsibilities
- Manages discharge reference tool/discharge calendar to ensure notification and documentation of daily discharges.
- Manages post-discharge tracking tool to ensure quality assurance measures for performance improvement.
- Manages and updates resource guides/lists per county and consumer as needed.
- Review and facilitate safety planning with former patients who require additional support.
- Documents all services provided in the patient's EMR.
- Supports department in administrative duties, auditing, and tracking data for QAPI and compliance.
- Supports Nursing department with related tasks, and provide direct patient care when requested.
- Performs other duties, as assigned.
- Demonstrated commitment to working collaboratively as well as possessing the skills to lead, influence, and motivate others.
- The Post Discharge Crisis Coordinator will follow the expectations of the Zero Suicide protocol per Corporate's guidelines, and will also support in a hybrid role the hospital's nursing department.

Compensation
- LVN/LPT $35.00-44.19 Per Hour RN $54.00-68.18 Per Hour REPORTS TO: Director of Quality Assurance and Performance Improvement.
