About the role
- Ennoble Care's clinicians go to the home of the patient, providing continuum of care for those with chronic conditions and limited mobility.
- Ennoble Care offers a variety of programs including, remote patient monitoring, behavioral health management, and chronic care management, to ensure that our patients receive the highest quality of care by a team they know and trust.
- This on-site role is responsible for ensuring the delivery of high-quality healthcare by managing and leading a team of Care Coordinators in partnered dialysis clinics in the Tulsa, OK area.
Responsibilities
- Demonstrates a professional, compassionate bedside manner with a patient-centered approach and the ability to build rapport in a clinical setting
- Excellent organizational and time-management skills with the ability to manage multiple priorities effectively
- Ability to cultivate and maintain positive, collaborative relationships with care coordinators, providers, facility staff, patients and their families
Requirements
- Travel between facilities as required
- Proficiency in Microsoft 365, including Outlook, Word, and Excel
- Legally authorized to work in the United States of America
- Active, unrestricted LPN license in the state of Oklahoma
- At least 2 years of management experience in a healthcare setting
Nice to have
- Dialysis-specific certification (CDT, CHT, CCHT, CCHT-A)
- Highly preferred: Dialysis-specific certification (CDT, CHT, CCHT, CCHT-A)
- Highly preferred: Minimum one (1) year of experience working directly with hemodialysis/nephrology patients in a dialysis facility
Compensation
- Starting hourly: $22/hr
