Responsibilities

  • You will require travel to complete in-person visits with members in different environments to support holistic assessment and care coordination.
  • Develop and manage member-centered care plans with measurable goals and outcomes
  • Coordinate care across providers, community support systems

Requirements

  • 2+ years of experience working as a case manager or care manager
  • Experience working with adult populations
  • In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs.

Nice to have

  • 3+ years of care coordination or community-based experience
  • Managed care experience
  • Case Management Certification (CCM or similar)
  • Motivational Interviewing training
  • Experience working with Pediatric populations and their families
  • Bilingual (English/Spanish)
  • Reside in Eastern Kentucky ( strongly preferred)

Compensation

  • The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
  • The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
  • $71,100 - $97,800 per year
  • This job is eligible for a bonus incentive plan.
  • This incentive opportunity is based upon company and/or individual performance.
