About the role
- Serving as the primary point of contact for participants, this position ensures the comprehensive delivery of case management services for the Capital Area Workforce Development region under the NC PROWD initiative.
- Conduct individualized assessments and determine program eligibility.
- Please Note **This position is grant‑funded through October 2028**.
Responsibilities
- Complete participant intakes and develop tailored service plans.
- Coordinate supportive services-including basic needs assistance, transportation, employment support, and housing resources-to help participants overcome barriers to reentry.
- Collaborate closely with community organizations, workforce partners, and federal correctional facilities to maintain a consistent and streamlined referral and continuity‑of‑care process.
- Maintain accurate case notes, documentation, and performance data in compliance with program and grant requirements.
- Advocate for participants to secure needed services and support.
- Work within a multi‑disciplinary team to support decision‑making, troubleshoot barriers, and implement service strategies.
- Assist clients with navigating complex systems, resources, and community supports.
