Responsibilities
- Develops, documents, and executes a rational, safe, evidence-based, cost-effective patient-centered plan of care with patient's PCP and members of the care team utilizing national guidelines or established protocol.
Requirements
- Current valid licensure in the state of Washington and/or Idaho required.
- Pharmacists must be able to move around the facility between 1/3 and 2/3rds of the day, while sitting at a workstation occurs under 1/3rd of the day.
- Education/Experience: RPh or PharmD from accredited school of pharmacy.
Nice to have
- Minimum of 1 year experience in a clinic setting as a clinical pharmacist or residency experience preferred.
- Prior non-profit experience preferred.
- Ambulatory Care experience, Board Certification and/or Certified Diabetes Educator (CDE) preferred.
- Experience with process improvement and supervisory experience a plus.

Compensation
- $107,300.00 - $162,301.00
