Responsibilities
- Support youth in building the skills, confidence, and independence needed for adulthood.
- Provide individualized case management and ongoing support for your assigned caseload.
- Develop, implement, monitor, and evaluate Safety Plans and Service Plans.
- Work collaboratively with youth, families, caregivers, service providers, and community partners to ensure coordinated care and support.
- Maintain accurate documentation, case notes, and professional reports.
- Advocate for youth while promoting strengths -based, person- centred supports.
Requirements
- Successful applicants must have:
- This role requires the ability to:
- Participate in recreational activities and physically engage with youth as required.
- A minimum of two (2) years of experience supporting children, youth, or young adults.
- We're looking for someone who has:
- A Diploma or Bachelor's Degree in Child and Youth Care, Disability Services, Social Work, Psychology, Community Rehabilitation, or a related field.
- Experience working with individuals experiencing developmental disabilities, mental health challenges, addictions, trauma, attachment concerns, or barriers to community resources.
- Knowledge of community resources available to support youth.
- Excellent organizational skills and the ability to write professional reports and documentation.
- Confidence using computer systems and electronic documentation.
