About the role

The Clinical Pharmacist is responsible for providing medication therapy management services in collaboration with other providers to assure that drug therapy is safe, effective, efficient, and cost-effective.
The Pharmacist will provide training and education to pharmacy residents, students through practical and didactic experiences.
The pharmacist ensures that patient care is maintained through compliance with the department's policies, rules and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies.

Incumbent acts independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines with support from the Lead Pharmacist and Pharmacy Director.
The Pharmacist performs both clinical and distributive tasks as necessary.

◾ Pharmacy degree from an accredited college of pharmacy ◾ ASHP accredited pharmacy (PGY-1) residency training preferred
◾ LTC consultation experience is preferred.
◾ Working knowledge of clinic-based operations preferred.
◾ Certification by Board of Pharmacy preferred
◾ Licensed to practice in the State of Wisconsin.
