About the role

Since 1997, Silverado has been delivering exceptional care to individuals with neurodegenerative conditions and those facing life-limiting illnesses through our Hospice services.
This role is essential to ensuring quality care and supporting patients throughout their end-of-life journey.
Supporting documentation is maintained by Silverado and is available for review upon request to ensure compliance with California law.

Develop detailed Plans of Care for assigned patients, ensuring all services are identified and met
Collaborate with the Interdisciplinary Group, patient, family, and attending physician to coordinate care
Perform field case management and other duties assigned

Ability to lift up to 50 pounds
Valid driver's license, good driving record, and reliable transportation required
Minimum of 1 year experience as an RN required; hospice experience preferred
Registered Nurse with a valid state license
Must clear criminal background check, physical, and drug screening
Willingness to drive to patient locations and work outside regular hours as needed
