About the role

  • Reporting to our Senior Medical Director the Geriatric Advanced Practice Provider plays a key role in delivering high-quality, patient-centered care to an ageing population.
  • This role offers a unique opportunity to advance preventive, geriatric-focused care for seniors in under-resourced communities.
  • This position brings specialized geriatric expertise into community health centers that traditionally do not have dedicated geriatric services, enhancing both the level and quality of care delivered.

Responsibilities

  • Develop individualized, patient-centered care plans in collaboration with patients, caregivers, and interdisciplinary teams.

Requirements

  • Experience in geriatric care required
  • Familiarity with EHR documentation and Medicare compliance
  • Experience working with interdisciplinary or care management teams
  • Fluent Spanish speaker required
  • Master's degree or higher from an accredited Nurse Practitioner or Physician Assistant program

Compensation

  • The provider will deliver billable clinical services in collaboration with contracted health centers, including conducting Annual Wellness Visits (AWVs) and Advance Care Planning (ACP) discussions, while also contributing to program design,

  • Perform thorough assessments with a focus on chronic disease management, functional status, cognitive health, fall risk, and medication management.
