About the role
- Thrive By 5 is currently hiring Service Coordinators for the Early Intervention Program in New York City.
- Essential Duties and Responsibilities: The essential duties and responsibilities are not limited to the following: Assist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP.
- Ensure the IFSP outcomes and strategies reflect the family's priorities, concerns, and resources; and that changes are made as the families/child's needs change.
Requirements
- Ability to effectively communicate with clients and families via phone and email.
- Perform related duties as required.
