  • JOB DESCRIPTION: SUMMARY OF POSITION Serves as an integral member of the multi-disciplinary team and a core facet in the continuum of care for Behavior & Primary Healthcare Coordination (BPHC)-enrolled clients.

Responsibilities

  • Provides support across the phases of care that is responsive to the changing needs of the persons served.

Requirements

  • High School diploma or GED required.
  • Years of Experience
  • Perform all required job duties and responsibilities in accordance to CCBHC Standards.
  • Education Level
  • High School diploma or GED required. BA/BS from a four-year college/university preferred. Equivalent work experience may be substituted.
  • Two years of direct service experience working or volunteering in a human service organization.
  • Certification/Licensure
  • Task Percentage of Time/Frequency
  • Standing - 20%
  • Sitting - 80%
  • Squatting - occasional
  • Driving - occasional
  • Kneeling - occasional

Nice to have

  • BA/BS from a four-year college/university preferred.
