About the role
- The Service Coordinator fulfills all duties of the Designated Coordinator in a Rule 245D program licensed by the MN Department of Human Services.
- This position provides case management services, direct care and instruction for adults with developmental and other disabilities in a day training and habilitation program.
- Working directly with persons in the program throughout the day and providing leadership/work direction to other program staff to ensure that individualized plans are properly implemented and that people served in the program receive quality services.
Responsibilities
- Performing all aspects of internal case management, developing individualized program plans, acting as a member of the support team for persons served, acting as the liaison with each participant's family and residential provider.
Requirements
- and has experience with MN Statues 245D.
- Must be able to frequently stand, bend, stoop and reach and push manual wheelchairs used by adults.
