About the role
- Researches, evaluates and recommends resources to meet medical and non-medical needs of patients and families.
- Responsible for supporting decisions that impact health care outcomes, resource allocation and customer experience.
- Acts as a primary contact for the care team to assist in navigation and complexity management.
Responsibilities
- Position will support cancers across the care continuum
- Provides care management support to panel of patients who require specialty services integrated and in support of their overall plan of care.
- Provides care management and overall plan of care support to a panel of patients who require integrated oncology services.
- Actively engages in care team and departmental meetings to support interdisciplinary collaboration.
- This includes well-being dollars, dedicated well-being navigators, and many programs, activities, articles, videos, personal coaching and tools to support you on your journey.
Requirements
- Bachelor's degree in nursing
- 2+ years inpatient or outpatient oncology nursing experience
Nice to have
- 3+ years nursing experience in clinical specialty area (i.e., Oncology, Hematology, Care Coordination or Care Management)
- Progressive experience in performance improvement and/or program development
- Licensed Registered Nurse
- MN Board of Nursing required upon hire
- Licenses/Certifications
- Licensed Registered Nurse - MN Board of Nursing required upon hire
- Must meet at least ONE of the requirements below:
- Current BLS certification from the American Heart Association
Compensation
- $46.42 to $64.22 per hour
- The pay described reflects the base hiring pay range.
- Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
