About the role
- Engages and provides care and support to Behavioral Health clients, patients, and/or families. Performs psychosocial assessment and care management. Plans, implements, and documents care and treatment. Collaborates with clinical and non-clinical team members and supports multidisciplinary team. Completes clinical triage assessments, manages clinical
- documentation, and patient schedules, makes direct referrals for patients when needed, conducts brief interventions if needed, crisis intervention, safety/ risk assessments, and supervision of interns.
