About the role

We ensure our caregivers have access to the right resources and guidance in all areas of outpatient care, so they feel confident and prepared to care for their loved ones at home.

Conduct home visits to assess and create personalized care plans.
Train and support caregivers, ensuring they can meet their loved ones' needs.
Maintain compliance with all relevant regulations and standards.

Documenting in an accurate and timely manner; maintain a clinical record for each morbidity patients.

What You Will Bring (Required) Bachelor's degree required; preferably in social work, human services, or a related field.
Ability to travel (by automobile) within assigned geographic area; valid driver's license and automobile insurance is required.
Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc).
Additional state-specific qualifications may be required.

Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
Monitoring the health status of all members to ensure that all needed care is properly delivered.

This role requires frequent travel.
You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).
