Responsibilities
- Collaborate closely with the RN Case Manager and interdisciplinary care team.
- Document all patient care accurately and in a timely manner.
- Identify safety concerns and help implement interventions that reduce hospitalizations and emergency room visits.
- Deliver compassionate, patient-centered care while maintaining compliance with all clinical and regulatory standards.
Requirements
- Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program
- Minimum of one (1) year of nursing experience in a clinical setting
- Must be able to lift, push, pull, and support 50-100 pounds, including assisting with patient positioning, transfers, and moving equipment
