About the role

  • Please Do Not Apply if you do not qualify.
  • The CCC shall ensure appropriate client referrals, authorizations, and transitions to new levels of care (LOCs).
  • Additionally, the CCC will ensure follow-up and clinical care coordination of services and actual connection to other services for the client.

Responsibilities

  • Working with the primary counselor, case manager and referral sources to develop an outcomes-focused, strengths-based person-centered service plan that includes both formal and informal services and supports.

Requirements

  • You must have a RN, LGSW, LICSW, or LPC to Qualify.
  • Valid state driver's license, proof of auto insurance and ability to utilize personal car for travel within the district.
  • The remaining hours per week are designated for administrative duties such as completion of clinical documentation, clinical supervision or consultation, required trainings, accreditation activities, etc.
  • Administrative duties such as completion of clinical documentation, clinical supervision or consultation, required trainings are pro-rated, if provided at all pursuant to fully executed contractual agreement.

Skills

  • Working knowledge of computers and electronic medical records systems.
  • Excellent negotiation, interpersonal, analytical, oral and written communication skills.
  • Ability to effectively interact with community leaders, professionals and staff.
  • Knowledge of professional standards and ethics for various disciplines.
