About the role

  • The Psychiatric Care Coordinator / Nurse (RN) plays a key role in supporting behavioral health services through clinical care coordination, patient follow-up, and interdisciplinary collaboration.
  • This position focuses on ensuring continuity of psychiatric care, supporting patients across levels of treatment, and reducing gaps in care through proactive outreach and coordination.
  • In addition, this role will be responsible for medical case management of PCMH target populations whose needs often bridge psychiatric and physical medicine.

Responsibilities

  • Monitor and manage active psychiatric caseloads, including outreach to patients who have missed appointments or have not been seen within 90 days
  • Obtain updates from IOP providers and support patients in re-establishing care following program completion
  • Support utilization of lab services, including urine toxicology (UTOX) and pharmacogenetic testing (e.g., GeneSight)

Requirements

  • At least 1 year of clinical experience
  • Experience maintaining a trauma-informed space
  • Ability to relate
